It is traditionally limited to the size of a single sheet of
paper.

answers the following questions.
What is the problem?
What is the root cause of the
problem?
What are potential solutions
to the problem?

To make the form easier to
print, organize, and view, you can create a series of
A3
subforms. For example, use an A3 header, an A3 current state deficiencies, and
an A3 root cause analysis.
Include results from VSMs,
Pareto charts, bar charts, and so on to document the current state and root
cause analysis.
Identify the metrics to
track the improvement progress.
Identify the tasks required
to implement the plan.
