Have you considered potential problems that can occur when an improvement changes the process?
Can you describe the nature and scope of the improvement?
Has the improvement plan been implemented, not just planned?
Have you updated the tools to reflect the changes from the improvement?
Have you verified that the measurement systems are still adequate after improvements have been made?
