Use the A3 form to tell the story of a problem, its analysis, and its solution. It is traditionally limited to the size of a single sheet of paper.
The A3 answers the following questions.
  • What is the problem?
  • What is the root cause of the problem?
  • What are potential solutions to the problem?

How-to

  1. To make the form easier to print, organize, and view, you can create a series of A3 subforms. For example, use an A3 header, an A3 current state deficiencies, and an A3 root cause analysis.
  2. Include results from VSMs, Pareto charts, bar charts, and so on to document the current state and root cause analysis.
  3. Identify the metrics to track the improvement progress.
  4. Identify the tasks required to implement the plan.
  5. Include results from VSMs, process maps, charts, and so on to document the future state.
  6. Revise as needed.
