The Meeting Minutes form answers the following questions.
  • What was discussed at a meeting?
  • What decisions were made?
  • Which tasks were assigned and when are they due?

How-to

Try to issue the minutes on the same day or at least within 24 hours of the meeting.

  1. Set an agenda and list of attendees before the meeting.
  2. During the meeting, take minutes, including who attended, tasks assigned, and the dates that they are due.
  3. Complete the form with meeting information. You may want to have the meeting leader approve the minutes before they are official.
